Now that you have decided that you are ready for a canine partner, you will need to have two forms completed and submitted to us.
Step One
Please print and have your therapist complete your Medical Form

Step Two
Please print and complete your Application Form,
APPLICATION FORM
Step Three
Return both documents to CVSDU by either:
1. Scan and email them to us at application@cvsdu.ca

Lastly, please be sure to provide a copy of your military ID, veterans card, or first responder ID.
